Applicants must be members of the Chaplain Alliance. The membership fee is $50 for the first year and $150 per year thereafter.

• Current Resume, Transcripts (M.Div or Equiv.)

• Ordination Letter/Certificate (Current Faith Group/Assembly)

• Three Professional Reference Letters

• CPE Certificate(s) (If completed)

• Resident Addresses (Last 5 years)

• Personal Faith Statement - (Include why you wish to join Way Maker Chaplain Alliance)

• Application Fee is 150.00 (Non-refundable, includes membership fee $50)
